Finally a concise easy to use, 'self-guided ' form specifically for use during a CODE BLUE. This two sided heavy stock foldable form allows the responders to document the event from beginning to end, with special attention to times, rhythms, compressions, defibrillation, IV access, advanced airway, pharmacology and H’s and T’s.
This form follows the AHA guidelines for in-field CODE protocols and allows the responder to record critical data for continuing care, termination of efforts and final PCR preparation.
